Tracy Hasler, ANP, has always had a special interest in caring for geriatric patients. She serves in a dual role of seeing patients in the clinic, as well as rounding on residents in nursing homes and assisted living centers.

In the long term care facilities and assisted/independent living centers, Tracy provides primary care. Instead of her patients traveling to her office, Tracy goes to her patients at the long term care or assisted living facilities and performs the same type of care that she would perform in the clinic, in the comfort of their home instead.

Tracy collaborates with a team of providers who are energetic, professional, and dedicated to the practice. This integrated relationship offers more services and support to the patients and their families.
